Students REQUIRING Online PERT Test Facilitation:
- First time applicant – Complete the online application, print the “Consent Page”, parent and student will both sign it. Parent signs this form as the Counselor as well as the parent.
- Be sure to select the correct Term of Entry (Summer 2021 – 202130 / Fall 2021 – 202210).
- Student can complete the application and receive a SJR State College Student ID Number.
- Students requiring online testing facilitation will submit their completed Consent Page to their Counselor for approval with the request for expedited processing to test online.
- ONLY APPLICATIONS FOR TESTING FACILITATION WILL BE ACCEPTED BEFORE February 15.
- Student can now contact SJR State Testing Department at 386-312-4228 or [email protected] and schedule a PERT online, if necessary, through Canvas or on one of the Campuses for $20.00.
Students NOT requiring Online PERT Testing:
- First time applicant – Complete the online application, print the “Consent Page”, parent and student will both sign it. Parent signs this form as the Counselor as well as the parent.
- Be sure to select the correct Term of Entry (Summer 2021 – 202130 / Fall 2021 – 202210).
- Student can complete the application and receive a SJR State College Student ID Number.
- Student prints the SJR State DE Online Signature Page, student and parent fills it out, signs it, and has it ready to submit along with their test scores when their high school is ready to accept it.
- When Schools are ready to accept applications, the student is ready to submit.
- SJR State Dual Enrollment will accept all applications effective February 15, 2021 – BE SPECIFIC WITH TERM STUDENT WISHES TO BECOME ACTIVE.
